Need to know how to include help information for each custom field user, Ex. In Phone field need to indicate country prefix need to be indicated.

Hi,
there are different ways:
1) Fill html editor in edit field page, this will become a Tip (screen1 and screen2)
2) Each field have a placeholder option (screen3)
3) Add a description with HTML delimiter field type after or before your phone field (http://docs.easy-profile.com/index.php/article/html-delimiter-field-type)
