Hi,
1. How do I make the registration name/field wider?
2. The form is not very organized as their is no separation between the rows. How do I add lines between each of the rows.
3. I am using checkboxes with a simple "yes" or "no" and I have "no" as the default. The default "no" is not checked.
4. How do I add a column label "Name" or something like that?

Easy Profile extends Joomla User Management so registration and edit profile page are the same of Joomla.
The width and space between fields are managed by your template, you can configure these by adding some CSS rule. If you need help please send me your site link (you can use the form below the reply in "Site Details" tab)

if you have set option like these:
1|No
2|Yes
you need to put in default value parameter this: 1
TIP: For Yes/No fields we recommend to use radio instead checkbox field, with checkbox you can select Yes and No
