Hi,
this is simple:
- Menu Module for links
- HTML Custom module for name and avatar

In HTML Custom module we use Content Plugin ( docs at http://docs.easy-profile.com/index.php/article/content-plugin ), with this simple code:

Hi {user name} {user avatar_mini}

NOTE: to trigger Joomla content plugins the module need to have enabled the "Prepare Content" parameter.

To create Logout menu item we have used our custom code, but this is not necessary, you can use a simple extensions like "Quick Logout" (http://extensions.joomla.org/extension/quick-logout), it's free.

Hi, i've tried your solution but i've a problem when a user has no profile image, it's very big !
Take look attachment.. how can i solve it ?

Hi,
you can mange this with 2 alternatives:
1. Add a CSS code to set size of all avatar images
2. Instead use {user avatar_mini} you can add HTML like this:

<img src="/{user avatar_mini raw}" height="50" width="50" />

{user avatar_mini} return HTML avatar
{user avatar_mini raw} return avatar URL
