Hi,
from here works: http://fantasypoolfreak.com/index.php/component/users/

your registration module breaks our code, Easy Profile create fields for Firstname and Lastname (you can set name style from Easy Profile option), but Joomla require name core field. Easy Profile make name field as hidden and assign a default value, in your module missing default value.
