I'm looking for an extension that I can use at time or user registration. I want the user to certify their age as on condition and then there is one other option, for now that if they request it, they get assigned to a different user group.

Will Easyprofile do that?

Yes this is possible with conditions, with conditions you can assign usergroups and hide/show several fields.

On the registration form, I would put a simple checkbox for them to check with the label "I certify I am over the age of 18". If they check that, they would go into a different group from those who do not check the box. Also, I may have checkboxes for users to check requesting various features and they could be put into other groups depending on what they checked.

Easy profile allow you to add/remove user from usergroups with simple condition, but I do not know what you mean by certify their age.
